Personalised Support Plan
Care Professionals provides in home and community support to people with disabilities and together we will discuss your needs and develop a tailored care plan.
Care Professionals also have staff trained in mental health first aid and work closely with people with mental health concerns and their families and other agencies.
We provide a mentoring program to young adults with a disAbility, which includes community access and activities of daily living skills. We have been working closely with Case Managers of Oz Child (Mornington) in the development of this popular group program.
Safety and Security
Our carers have current working with children checks and police checks, and attend ongoing training both in house and externally.
It is our responsibility to help and protect the most vulnerable in our community. Our team is there for you.
We are Your Advocate
When Care Professionals CEO, Angela Dunleavey, worked as a Home and Community Care Assessment officer she lobbied for people with mental health issues to receive in home support such as home care, cooking and shopping supports and monitoring checks. We are also here to assist you to advocate for your rights.
We understand the constraints of funding and we will work with clients and families to ensure the best mix of services to meet your needs.
We are here to Support Together
We welcome the involvement of your family and carers. We can provide support and information to your primary carer. For example the government funded carers allowance, counseling, respite support and additional funding.
