Safety and Security
Safety at home is vital. We will provide you with a home safety check and assist you to gain access to appropriate support to ensure your home is as safe as it can be.
All members of the Care Professionals team have a current police check, a work reference check and are trained to identify and report situations which may not be in your best interests.
Personalised Support Plan
Together we will discuss your needs and develop a tailored care plan. This will include your social and spiritual needs as well as your personal and household needs. You are very much an individual and we are passionate about ensuring you have the right to live as you choose, and where you choose.
Know Your Entitlements
We will explain and assist you to gain access to government funded support that can save you money and improve your quality of life.
We are your advocate.
We understand the constraints of funding and we will work with clients and families to ensure the best mix of services to meet your needs.
We are here to Support Together
We welcome the involvement of your family and carers. We can provide support and information to your primary carer such as the government funded carers allowance, counseling, respite support and additional funding, plus assist with planned residential respite. We can help you and your family to identify the most appropriate permanent placement should this be necessary.
